What should I expect at my first appointment?

Please complete your new patient paperwork online using the link sent via text message, or print the forms and fill them out at home ahead of time. Arrive 15 minutes early if you haven't completed it in advance. Upon check-in, please have your ID and insurance card(s) ready so we can make a copy.

How do I order/request refills?

Accepted Answer

The fastest way to receive refills is to call your pharmacy. They will send an electronic request to our office, and they can be filled in a timely manner.

You can also leave a message on our office voicemail for prescription refills. However, these requests may take 24- 48 hours to process. When leaving a message, please speak clearly and slowly. Leave your name, date of birth, medication needed, and pharmacy preference. Multiple phone calls will delay the process.

Why was my pharmacy request denied?

Accepted Answer

There could be several reasons:
If a provider is new to our office, the pharmacy requests may be going to their old practice. In this instance, a call to our office is appropriate.
Prescription refills can only be approved if you have seen the provider within the last year.
There may have been a change in your prescription.

My medication is too expensive. What can I do?

Please contact our office if your medication is too expensive. We may be able to direct you to cost saving options for high priced medicines, or the provider can prescribe something similar that is more cost effective.
